The New England Division of General Practice runs a diabetes program, which is centred on a voluntary database and recall system for the annual review of adults and children with diabetes. The aim of the Diabetes Program is to improve the health and well being of people with diabetes. It provides a co-ordinated approach to diabetes management and centres around a comprehensive annual review with the GP.
The Division sends an annual recall letter to participants in the program, which includes a request form for pathology needed to undergo the diabetes review. The review enables the participant to obtain the best and most up to date information on the care of their diabetes and will ensure that any possible complications are identified and treated early.
The Division also provides a GP Management Plan template, which meets best practice guidelines for a diabetes annual review. Once a copy of the completed GP Management Plan is received at the Division, the data is inserted onto the database and a cumulative report is produced and sent to both the GP and the patient. The Division also offers some dietetic and diabetes education services to remote areas, according to need.
