The Heart Health Program aims to improve the clinical management and reduce risk factors in people with or at high risk of developing cardiovascular disease. The Heart Health Program goals include:
· Rehabilitation after a cardiac event;
· Regain/ improve functional capacity and quality of life;
· Primary & secondary prevention with lifestyle changes and risk factor modification;
· Improve patient care in general practice;
· Implement best practice guidelines;
· Assist GPs with risk factor monitoring and management.
The Heart Health Program offers:
· A Cardiac Rehabilitation Program in Armidale;
· A Cardiac Rehabilitation Program in Glen Innes & Inverell;
· Maintenance Exercise Classes in Armidale (twice weekly for one hour, Wednesdays & Saturdays, open to the public);
· Referral for graduates to community exercise program;
· The Heart Health Register – a reminder/recall service encouraging participants to attend an appointment with their GP, in order to check risk factors for heart disease, on an annual basis. Available for participants/GPs across the Division.
The Cardiac Rehabilitation Programis based on exercise, education, support and risk factor modification. It is designed for patients with:
· Coronary artery disease;
· Myocardial infarction;
· Cardiac surgery (eg. CABGS, valve replacements);
· Angioplasty & stenting;
· Chronic cardiac failure;
· Significant cardiac risk factors.
Enrolment to the program is via referral by GP, Physician, or tertiary hospital. Patients are assessed for risk factors and provided with recommendations for home and group exercise, and education, by program GPs and Physiotherapists. All patients are assessed as to their eligibility for a Team Care Arrangement by the GP. A GP referral is required for all participants.
The program in Armidale provides group based exercise and education sessions, held twice weekly at the Rehabilitation Gym, Armidale Rural Referral Hospital, Armidale.
The programs in Glen Innes and Inverell, provided by HNEAHS, are held in blocks, 3 - 4 times during the year. NEDGP supports a GP to provide GP Question and Answer education sessions in Inverell.
