The Memory Assessment Program is a unique initiative that was implemented with the view to offering a streamlined approach for GPs in the care of their patients experiencing memory problems. The program is intended to support people with early to moderate stages of dementia and their carers from diagnosis through the course of the disease. The process aims to reduce the stress for the client and their carers by helping them to increase their awareness of services relevant to their needs for the present and future.
The program links GPs and their patient’s with dementia to specialised medical and neuropsychological services and eliminates the need for patients to contact the many agencies who may provide services. Case conferences and regular reviews ensures the optimisation of aged care services through local agencies such as, ACAT, Community Options, home care, respite and hostel providers.
The program is offered to GPs in Armidale, Uralla, Walcha, Guyra, Glen Innes, Inverell and Tenterfield.
The position of Co-ordinator is the focal referral point for GPs and makes the appropriate links between services and referrals to specialist services.
To access the program the person needs a GP referral by completing the Memory Assessment Program Management Tool and Folstein’s Mini Mental State Examination/Clock Drawing Test. You then need to write a referral letter to Dr Gerry De Gabriele, our local Geriatrician, and fax the relevant forms to the Coordinator on 6771 1170. Once the referral is received at the New England Division of General Practice, your patient will be contacted and the program will be explained to them. If your patients prefer Dr De Gabriele can visit them at their home (if they live in Armidale). Otherwise we can arrange for them to see Dr De Gabriele in the Clinic building at Armidale Hospital. Dr De Gabriele generally sees Inverell and Glen Innes patients in those centres if possible.
